Borderline is usually treated with surgery and followup. The chance of recurrence is minimal. I would ask the doctor to explain exactly what is meant by microinvasion, what type of follow up he suggests, if he has had experience with other borderline patients and how they have progressed.

Objective The aims of this study were to evaluate the rate of recurrences in borderline ovarian tumors (BOTs) with microinvasion and to evaluate the possibility to enlarge fertility-sparing surgery in this group of patients. Borderline tumors with intraepithelial carcinoma and/or microinvasion provide evidence that these tumors form a morphologic spectrum with individual types representing steps in the sequence of mucinous carcinogenesis in the ovary . Cytologic atypia in microinvasion is mild to moderate, similar to adjacent borderline tumor. Presence of severe atypia warrants the diagnosis of microinvasive carcinoma. Mucin extravasation with inflammatory response and histiocytes is associated with gland rupture and not diagnostic of microinvasion.

2 Mucinous tumors 2019-03-01 Serous borderline tumors of the ovary: a long-term follow-up study of 137 cases, including 18 with a micropapillary pattern and 20 with microinvasion. Am J Surg Pathol. 2002 ; 26 : 1111 –28. 5. Serous borderline tumor of the ovary initially was described in 1929 by Taylor. 1 Current information suggests that women with Stage I serous borderline tumors have an excellent prognosis, with a disease free survival rate approaching 100%. 2 Standard therapy for Stage I serous borderline tumors is comprised of surgery alone.
Microinvasion may be represented by a single focus or multiple
The upper size limitfor microinvasion has been arbitrarily set at 10 mm2 (10). 1 Mar 2012 In 12 series of serous borderline ovarian tumours with microinvasion (n=133), 20 patients had recurrence (15%), including 35% (seven of 20) 29 Dec 2017 Keywords: borderline ovarian tumor; BOT; recurrence; survival; treatment Stromal microinvasion was defined as the presence of stromal 31 May 2000 Ovarian tumors of low malignant potential (LMP) would benefit from a new name The name “borderline ovarian tumors” suggests that these tumors and “ microinvasion” as predictors of progression; the M. borderline tumor (SMBT) is thus a synonym for MEBT-M, MMBT, and EMBT. invasion of < 5 mm in greatest dimension is defined as microinvasion, regardless of number of foci; tumors with microinvasion are considered SMBT (“SMBT with microinvasion”). 2018-08-01 Two of 13 (15%, 95% CI 8.7, 21.3) patients with microinvasion died of recurrent invasive cancer, whereas only 1 out of 113 patients without microinvasion died of recurrent borderline tumor (OR 20.4, 95% CI 1.2, 239). All 3 patients with an aggressive disease course and poor outcome had increasing CA 125 levels at the time of recurrence. Comments: Borderline clear cell adenofibroma with a 2mm focus of microinvasion in a 10 cm tumor. The remainder of the tumor was composed entirely of benign areas.
Conclusions: Advanced stage at diagnosis was the most important prognostic marker in patients with SBT.
Micropapillary tumor is a growth pattern of serous borderline tumor which shows proliferation of the tumor cells in elongated, thin micropapillae with little or no stromal support emerging directly from a lining of a cyst or from large papillae in a non-hierarchical pattern. With microinvasion: Foci of stromal invasion, measuring < 5 mm in the greatest dimension Degree of cytologic atypia is mild and similar to borderline tumor Areas of mucin extravasation with inflammatory response are not diagnostic of invasion
serous borderline tumors and serous borderline tumors with non-invasive micropapillary pattern [1].

Microinvasion is reported in up to nine per cent of mucinous borderline tumours of intestinal-type.1 Invasive foci may consist of single cells, small clusters, glands or foci of confluent or cribriform growth within the stroma (see Figure 6). Borderline is usually treated with surgery and followup. The chance of recurrence is minimal. I would ask the doctor to explain exactly what is meant by microinvasion, what type of follow up he suggests, if he has had experience with other borderline patients and how they have progressed.
